Thank you for your interest in joining the Gift of Life NexGen Registry! Complete this page and the next steps which include a brief health history and your contact details. We will then email you a link to complete a more detailed questionnaire as the next step in becoming a NexGen donor.

Our NexGen staff will process your registration and let you know if you qualify to become a volunteer donor. Next, you will have a physical to ensure you are healthy and can donate.
